Thank you for your interest in Talbot Park Baptist Preschool and Daycare. We are excited about this upcoming school year and the potential for your family to be a new addition to our program. To be considered for a spot in our program all forms must be completed and returned. Packets are sent out by the administrative office once a spot is offered. Once everything is complete your application will be reviewed by the administrative team. If your child is accepted into the program, then you will have 2 days to pay the registration fee and turn in the rest of the required paperwork to secure your child’s spot. If you are unable to turn in the rest of the necessary items in the 2 days following please make a plan with the office for finishing up all required documents. Turning in all paperwork does not secure your child’s spot until you receive an acceptance letter from the administrative team. Listed below are all items needed for the application process. • Getting to know you form (preschool only) 

• Infant/Toddler form (infants and toddlers only) 

• Potty training agreement (if applicable) 

• Playground form (preschool only) 

• Preschool Recommendation form (ages 2-5) (online form)

 

Once an acceptance letter is sent then the following items are needed to complete the application. 

• Copy of your child’s birth certificate

• Virginia Health Entrance Form which includes immunization records 

• Registration Fee Paid
