Thank you for your interest in Talbot Park Baptist Preschool and Daycare. We are excited about this upcoming school year and the potential for your family to be a new addition to our program. Once offered a spot in our program all forms must be completed and returned to finalize registration. Once your registration paperwork is submitted you will have 2 days to pay the registration fee. Listed below are all items needed for the application process. We look forward to working with your family.

• Tuition Agreement 

• Photo Release 

• Getting to know you form (preschool only) 

• Infant/Toddler form (infants and toddlers only) 

• Potty training agreement (if applicable) 

• Playground form 

• Copy of your child’s birth certificate

• Virginia Health Entrance Form which includes immunization records
